- Date of contest
- Tests needed (include grade level of students that will be taking the tests)
- Contact Person (include email address, phone number (day and night), and address
- If payment will be with Purchase Order include the P.O. number, shipping address and billing address. For documentation purposes send a copy of the P.O. to Leo Ramirez, 2908 Flamingo, McAllen, Texas, 78504 or fax to (956) 682-7281.
All testing materials and invoice will be mailed at least two weeks before the date of the contest. If materials are needed earlier please specify.
